Unit Overview

Description

This unit explores existing and emerging service delivery models for primary care, residential care and health services from the perspective of the user, practitioner and service. Models of care will be investigated with an exploration of team-based and interdisciplinary care, professional autonomy and an exploration of the complexities around consumer choice, fragmentation and integration of care and transitions of care.

Outcomes

Students are able to (1) able to discuss theories that guide practices of service delivery; (2) explore the challenges of change management to introduce a new service delivery model from the perspective of the community / organisation / practitioner / service user; (3) appraise the existing and emerging service delivery models in Australia and contrast requirements for their implementation; and (4) investigate the breadth of team and integrated care services and their implications for sustainability.

Assessment

Indicative assessments in this unit are as follows: (1) case studies and (2) case presentation. Further information is available in the unit outline.
