Unit Overview

Description

This unit is for people who are already credentialed to undertake medication management reviews and aged care onsite roles. This unit provides advanced application of the skills and knowledge in geriatric pharmacy. It focuses on building competencies in clinical practice.

Through the use of multiple case studies and case discussions, students will apply and demonstrate the practical application of their skills and knowledge for patient centred care.

This unit provides advanced application of the skills and knowledge in geriatric pharmacy. Through the use of case studies, students will demonstrate the practical application of these skills to provide person centred caret. Additionally, through the use of peer feedback through active participation in the discussion boards, learners will develop advanced skills to act as both a resource to peers and gain confidence disseminating the latest research, guidelines and procedures to other health professionals.

Outcomes

Students are able to (1) identifies high risk groups and groups with special needs and actively advocates to adapt the service to accommodate their needs; (2) demonstrates skills in facilitating learning for peers; (3) demonstrates competence in providing feedback; (4) discuss appropriate assessment and evaluation methods; (5) demonstrates ability to provide person-centerd care; (6) demonstrates ability to take undertake a clinical assessment of older people; and (7) communicates effectively with other health professionals.

Assessment

Indicative assessments in this unit are as follows: (1) case discussions; (2) discussion board participation; and (3) quality improvement activity. Further information is available in the unit outline.
