Unit Overview

Description

The unit will provide an understanding of people living with complex health needs (e.g. multimorbidity) for an understanding of the psycho-social, investigative and management options.

This unit will prepare learners to undertake Medication Management Reviews for people with complex care needs. Formally accredited with the Australian Pharmacy Council, people who have completed this unit will be credentialled to be able to undertake funded Medication Management Reviews.

Outcomes

Students are able to (1) identify relevant information required to assess medication regimens for people with complex health needs; (2) demonstrate an understanding of common health conditions in older people; and (3) assessment of medication regimens for people with complex health needs.

Assessment

Indicative assessments in this unit are as follows: (1) completion certificates from completion of online training modules; (2) case studies (each case study assessed as an equal proportion of the mark); and (3) discussion board interactions. Further information is available in the unit outline.

To pass this unit, a student must: (a) achieve an overall mark of 50 per cent or higher for the unit; and (b) achieve the requisite requirements(s) or a mark of 50 per cent or greater, whichever is higher and specified in the unit outline, for the completion certificates from completion of online training modules component.

Student may be offered supplementary assessment in this unit if they meet the eligibility criteria.

Professional recognition
This unit is accredited against the Australian Pharmacy Council's Aged Care for Medication Management Review Pharmacists. Once successfully completed, pharmacists will be credentialled to provide Home Medication Reviews (HMRs) and Residential Medication Management Reviews (RMMRs).
