Unit Overview

Description

The unit will provide an understanding of people living in residential aged care facilities to prepare pharmacists to work as pharmacists in aged care. Formally accredited with the Australian Pharmacy Council, people who have completed this unit will be credentialled to work as aged care onsite pharmacists in residential aged care facilities.

Learners will use evidence based materials to develop the the necessary clinical knowledge to work in RACFs. Principles of geriatric medication management will be covered including evidence-based approaches to undertake interventions to optimise medicines including deprescribing and medicines simplification.

Learners will be given practical training and tools to implement organisational change and undertake quality improvement activities. The Towards Organisational Culture Change (TOrCCh) Toolkit will be used as an evidence-based approach to support learners to undertake changes in the workplace linked to a quality improvement cycle.

Leveraging the Towards Organisational Culture Change (TOrCCh) Toolkit, participants will gain a systematic approach to effecting positive changes in workplace practices and undertake quality improvement activities to enhance resident care outcomes.

Outcomes

Students are able to (1) synthesis of the investigation, diagnosis and management of health conditions for people in residential aged care facilties; (2) describe the governance and process structures that apply in residential aged care facilties; and (3) design interventions to improve quality use of medicines in residential aged care facilities.

Assessment

Indicative assessments in this unit are as follows: (1) assignment; (2) clinical quality improvement activity; (3) case studies; and (4) completion certificates from completion of online training modules. Further information is available in the unit outline.

To pass this unit, a student must: (a) achieve an overall mark of 50 per cent or higher for the unit; and (b) achieve the requisite requirements(s) or a mark of 50 per cent or greater, whichever is higher and specified in the unit outline, for the completion certificates from completion of online training modules component.

Student may be offered supplementary assessment in this unit if they meet the eligibility criteria.

Advisable prior study
It is recommended that pharmacists are credentialled to provide medication management reviews prior to undertaking this unit.
If pharmacists are not credentialled to undertake medication management reviews, it is adviseable that they undertake PHCY5156 prior to enrolling in PHCY5157.

This unit is available online in asynchronous format of learning. It includes compulsory experiential learning.

Professional Recognition
This unit is accredited against the Australian Pharmacy Council's Aged Care for Medication Management Review Pharmacists. Once successfully completed, pharmacists will be credentialled to work as aged care onsite pharmacists.
